Care Coordination
Once litigation is complete, the implementation of the life care plan begins. Individuals on whose behalf the plan has been developed, often benefit from consultation and guidance from an experienced care coordinator to navigate health and disability service systems and to bring services into reality.
Like our life care planning, the Medical Legal Services Division’s care coordination (care management) are person-centered with individuals and their families at the center of strategy development and goal implementation. Based on The Coordinating Center’s time-tested model, our care management services are known for the following:

Onsite visits and regular telephone contacts

Regular and scheduled reporting to the client/funder

Cost-efficient solutions to the client’s needs

Ongoing contacts with community providers

Oversight of progress in meeting the plan’s goals

Mobilizing of appropriate community resources, including both eligibility and access
Many of our care management clients are beneficiaries of trusts established following the litigation process, including Special Needs Trusts. Trust attorneys and financial professionals in particular may benefit from a knowledgeable care coordination professional, who can be “feet on the ground” with client’s, to assure services are acquired, and monetary disbursements are made in a manner that are both time and resource efficient.
